Payment Methods

Payment is requested in full at the time of service. Only major credit cards are accepted including those that are Health Savings Cards with credit card emblems on them. DCG will keep your card on file in your electronic medical record so it is safe and secure. DCG will submit your Insurance claims electronically to your insurance provider. Every insurance provides different coverage and reimbursement rates.  The most likely CPT codes used are 90791 and 90837; your insurance will request this information when you contact them for benefits. I will provide you with an invoice with all the necessary information for you to file with your insurance.

Although I am considered out-of-network with your health plan, I am happy to submit it electronically to your insurance company on your behalf, if I can. It will save you the paperwork.  Once I file electronically your insurance will reimburse you. If you prefer, I can also provide you with an invoice with all the necessary information for you to file with your insurance.  We can discuss this in greater detail during your appointment.
